SLOAN SEMESTER TRANSCRIPT INFORMATION

To ensure that the credits earned in Sloan Semester will be transferred back to your home institution, you must take the following actions.

Students completing courses as part of Sloan Semester must take the initiative to request transcripts from the institutions in which you were enrolled. Federal law will permit institutions to forward transcript ONLY when a written request from the student has been submitted. The following information outlines the procedures you must follow to have transcripts sent to your home institution for coursework taken in Sloan Semester.

We have created a listing of all institutions that enrolled students in Sloan Semester below. You can click on the institution or institutions where you were enrolled and it will take you to information and instructions for requesting a transcript. A number of institutions allow you to make requests online, some have forms for you to complete, while others require that you mail or fax your request.

As a general rule, you will need to provide (and should have ready as you prepare to make your request) several items.

• your full name
• your home and/or current mailing address
• your social security number and/or student number
• the course(s) you were enrolled in (course prefix and number, e.g. ENG 101)
• the institution to which you want to have the transcript sent (your home institution and mailing address for the Registrar’s office)
• any other information requested by the providing institution.

Your request should include the following statement: “The course(s) I was enrolled in were part of the Sloan Semester program. Participating institutions agreed that transcripts would be forwarded to participating home institutions at no cost.”

If you wish to have a copy of your transcript, you may request the institution to forward a student copy to you. Please note that this will not be recognized as an official transcript.

Should you need to have transcripts forwarded to other institutions, employers or other entities in the future, transcript requests and procedures of the institution will govern those requests (which may include fees).

Please note that official transcripts will not be forwarded until after grades are posted and institutions process transcript requests. The amount of time it takes to forward a transcript can vary based upon a variety of factors (holidays, submission of final grades, beginning of new academic term, etc.) and you should allow upwards of one month for the request to be completed.
